1. Welcome Letter  It is our goal to help you feel prepared and ready to come to Camp Fernwood this summer.  Enclosed are our pre-camp dates, contact information and a few other things to make sure you feel welcome.  
          
2. Arrival Dates  This form lets you know when you need to arrive at camp if you are taking the WSI, Lifeguarding Course, or are arriving at the regular start of orientation.  Some dates are still TBA but the information on this page lets you know what to expect for the courses.

9. Overseas Staff  This form has information specific to international staff.
   
   
10. Health Form A completed medical form based on a physical examination within the last 12 months is required. A physical done by your university is fine. If you have already had an exam in the last year, your physician need only complete the form. Let us know as soon as you can if you are not able to get a physical.

14. Employment Verification Form This document will indicate what documentation you will need to bring with you to camp to prove that you are eligible to work at camp. Please look at this form before coming to camp to make sure you have the appropriate documentation with you .
    
    
15. Social Security Form  For all new international staff who DO NOT already have a social security number from the United States.  Please download these forms and read the instructions.  You can bring these forms with you to camp if you do not understand what to do (1) download "directions for filling out social security form" | (2) download the actual social secrity form).
