    1. Welcome Letter It is our goal to help you feel prepared and ready to come to Camp Fernwood this summer. Enclosed are our pre-camp dates, contact information and a few other things to make sure you feel welcome.
    2. Arrival Dates 2010 This form lets you know when you need to arrive at camp if you are taking the WSI, Lifeguarding Course, or are arriving at the regular start of orientation. Some dates are still TBA but the information on this page lets you know what to expect for the courses.
    3. Travel Information Please fill out this form when you know your travel plans . Also, here are the directions from the airport, train and bus station.
    4. Bus Schedules Here is a list of departure and arrivel times, websites and contact information for buses from New York and Boston to Portland, Maine.
    5. Forms for Camp This is list of the documentation and forms you will need to bring to camp.
    6. Staff Packing List Here is a list of suggested items to bring to camp.
    7. Procedures and Services Here is a list of information about email, laundry, etc.
    8. Personnel Policies You should have received these with your contract.
    9. Overseas Staff This form has information specific to international staff.
    10. Staff Health History The Health History form is a 4 page document that you or a parent completes.  It allows you to provide us with pertinent information about your health history, immunizations, allergies, medications and medial providers. If you are under 18 years old, a parent signature is required on the form.
    11. Recommendation by Licensed Medical Personal The Medical Recommendations form  must be completed by your physician.  A physical examination within the last 12 months is required. A physical done by your university is fine. If you have already had an exam in the last year, your physician need only complete the form. Let us know as soon as you can if you are not able to get a physical.
    12. Emergency Form Please fill this form out and send it back to us.
    13. Background Check Please fill out this form.
    14. Voluntary Disclosure Statement Please fill out this form and mail it back to us.
    15. I-9 Employment Verification Form This document will indicate what documentation you will need to bring with you to camp to prove that you are eligible to work at camp. Please look at this form before coming to camp to make sure you have the appropriate documentation with you .
